I will not leave confidential or sensitive data in view of others who do not have a legitimate business reason to view the data and I will only share confidential data with persons having authorized access.
I will not discuss confidential or sensitive information or data in public places and will not remove confidential or sensitive data from my work site without College authorization.
I understand that unauthorized disclosure of confidential student information is illegal as provided in the Family Educational Rights and Privacy Act of 1974 (FERPA) and in the implementing federal regulations found in 34 CFR Part 99. FERPA is specifically incorporated into the Texas Public Information Act as an exception to records which are subject to disclosures to the public (Texas Government Code, Chapter 552). Unauthorized disclosure of confidential student information pertaining to health, including disabilities, is illegal as provided in the Health Insurance Portability and Accountability Act of 1996 (HIPAA). Illegal disclosure of confidential student data may result in fines to the College and the loss of federal funding.
I understand the data generated from this portal may have small samples which potentially could be used to determine an individual student’s grade in a course, or other data about a student. Therefore any reports generated should be kept confidential. If data is to be published or publicly shared, it should be in aggregated form or with categories containing small sample sizes edited out of the report.
I understand that my user ID and password are to be kept confidential; should I share this information, my access to TIPS will be revoked.
