All Shield of Faith Bible College students who accept scholarship assistance are asked to release potentially confidential personal and educational information to their scholarship donors. Such confidential information includes the student’s intended major, academic standing, hometown and information related to scholarship requirements. The Family Educational Rights and Privacy Act (FERPA) requires student consent to disclose personal and educational information to any individual. Students may grant their permission to release their educational information. In order to provide your scholarship donor(s) with the required information, we ask that you agree to the terms of this Scholarship Information Release Form (SIRF) by signing and returning this form to the Admissions Office. I hereby authorize Shield of Faith Bible College to disclose the above specified information in my scholarship, (financial aid) or academic records with my scholarship donor(s). I consent that this release will remain in effect until revoked by me in writing. I also agree to write a letter of appreciation to my scholarship donor(s) annually upon request from the Office of Admissions. You also consent to receive communications from us electronically. We will communicate with you by e-mail or phone. You agree that all agreements, notices, disclosures and other communications that we provide to you electronically satisfy any legal requirement that such communications be in writing.
