Please use this form to apply for graduation and receive your 8th grade or high school diploma.
 
⛔Wait to complete this form until the student’s high school transcript or 8th grade report card is complete. For high school students, this means all courses and credits are listed, all high school graduation requirements met, and the parent has carefully reviewed the transcript and given final approval.
 
Allow 3-4 weeks processing time after your student's grades have been posted to their transcript/grade report to receive the diploma in the mail.

High School Families:

❖ This form is required for graduation - a student has not graduated from Kolbe Academy until this form has been processed.

❖ Seniors must be enrolled full-time for senior year to be eligible for a Kolbe diploma & participation in Kolbe's commencement ceremony.
 

High School - Commencement Ceremony Registration

Kolbe Academy's 2025 Commencement Ceremony will be held on
Friday, May 22, 2025
Kolbe Academy is hosting an in-person and online commencement ceremony for the Class of 2025 on Friday, May 22 at TBD EST. Please register here to attend the event in person or online! An event schedule, cap & gown information, and other details will be sent to you in a confirmation email after you register.
 
Please note: Participating in our commencement ceremony is a separate process from graduating. Students do not need to have completed all of their coursework to join us for the commencement event. Students have until the end of 2025 to meet their diploma requirements and can still "walk" in the ceremony.
 
⛔Please do not submit this Graduation Application Form if your student has not completed courses and grades have not yet been reported and processed.

  • Choose 'Yes' if you have reviewed your student's complete transcript, (list of all grades, including transfer courses, taken during your child's high school career).
