Letter of Recommendation Request Form

This form is required for requesting a letter of recommendation, school report, or other admissions forms from Kolbe's advising and teaching staff. If you are applying to multiple schools or scholarships, you do not need to submit this form more than once. The results of this form will help your advisor or teacher write a strong, detailed letter of recommendation. The time and effort you place in thoughtfully and thoroughly answering these questions will be mirrored in the recommendation letter.

If you are requesting a letter of recommendation from a Kolbe teacher, please contact them directly to see if they are willing to write the letter on your behalf before adding their email address to this form.
