You do not need to submit this form unless you need to make changes to the courses you selected when you enrolled. If you need to check on which courses those are, please login to your Parent Portal.
You may also print a hard copy of the course of study forms (high school
) and mail it into us, although we highly recommend using this form for convenience. For any Online or Self Paced course changes, call or email admissions at ext. 2 or firstname.lastname@example.org.
