Completing this form is Step #2 in the Crowdfunding Process in Humble.

This form must be completed by Humble ISD teachers and staff AFTER the principal or administrator has granted you approval to post the request and implement the project if funded.

If you are requesting technology please review the Technology/Grants Guidance form. The policy can be found on our district’s website and is also linked on this form below.

This form will automatically be submitted to:

  • Melinna Lopez, the district’s resource development coordinator
  • Jaclynn Cabral, Fixed Asset Accountant
  • Your campus principal
