Thank you in advance for your willingness to help us determine the feasibility of an Annual Fund fundraising model for Elm Grove Elementary. Many schools in our area have transitioned to this fundraising model and away from sales-based fundraisers. So far, all schools who have tried this method have been successful using it.

Your participation on this survey is entirely voluntary, but necessary to our efforts. While certain fields are mandatory, you may always fill out the survey anonymously by typing "Anonymous" in the name box or n/a in fields you do not wish to answer. We do prefer that you disclose your name and contact info, so Foundation staff may follow up with any questions you may have or to gather further information.

Please fill out the survey as completely as possible and to the best of your ability. Each question is asked for a very specific reason. Questions about employer help us ascertain how many people may be matching gift eligible or would help us qualify for a corporate gift. Questions about job title help us in gathering info necessary for grants available from various trade/professional organizations. All answers will be held in the strictest confidence.
