PURPOSE. The purpose of this Agreement, effective upon submission, is to enable the player listed above to enroll in the roster of the ODL Team sponsored by Players Edge Training Center, pursuant to this purpose, I herby aggree to the terms and conditions set forth herein.
By checking the box below and as the arent or guardian of the player, I agree to all the terms of this agreement, which I have read and understand, and I authorize PLAYER'S EDGE TRAINING CENTER to charge (debit) my credit card for the one time fee of $750 (Seven Hundred Fifty Dollars). I understand my player is participating the ODL Team program and does so at his/her own risk. Players Edge and its owners, employees or agents, shall not be liable for any damage whatsoever arising from participation. Players and parents assume full responsibility for all injuries and damages which occur in or about any programs. He/She does hereby fully and forever release discharged hold harmless Players Edge, all associated facilities and its owner, employees, and agents from any and all claims, demands, damages or rights of action, present or future resulting from any person’s participation in the program. In addition, he/she agrees to follow the rules of conduct and play set by the Coaches. Failure to do so may result in suspension from participation. I do hereby authorize Players Edge and its assigns to utilize any and all photographs, pictures, video or other likeness of me, my athlete or anyone assigned guardianship to me or my athlete, as they deem appropriate in its promotional materials or team films.
STATUTORY NOTICE AS TO BUYER'S RIGHTS. ANY PERSON THAT AGREES TO THIS ON-LINE CONTRACT IS SUBJECT TO ALL CLAIMS AND DEFENSES THAT THE DEBTOR COULD ASSERT AGAINST THE SELLER OF GOODS OR SERVICES OBTAINED WITH THE PROCEEDS HEREOF, RECOVERY HEREUNDER SHALL NOT EXCEED AMOUNTS PAID BY THE DEBTOR HEREUNDER.
