By checking the box below, I agree to all the terms of this agreement, which I have read and understand, and I authorize PLAYER'S EDGE TRAINING CENTER to charge (debit) my credit card for my monthly dues, service fees or sessions based on this Agreement, plus any incidental or outstanding charges I may incur that are due and payable. I agree to keep a current email address on file and agree to accept emails. In enrolling at Players Edge, participant understands that he/she attending the programs and/or using Players Edge and the facilities does so at his/her own risk. Players Edge and its owners, employees or agents, shall not be liable for any damage whatsoever arising from any personal injury or property loss sustained by participant with his/her family in or about any programs on the premises. Participants and parents assume full responsibility for all injuries and damages which occur in or about any programs on the premises, He/She does hereby fully and forever release discharged hold harmless Players Edge, all associated facilities and its owner, employees, and agents from any and all claims, demands, damages or rights of action, present or future resulting from any person’s participation in any programs or use of the facility. In addition, he/she agrees to follow the rules of conduct and play set by Players Edge. Failure to do so may result in suspension from participation. Consent: I the parent or guardian/participant do hereby grant authority to the staff at Players Edge to render a judgment concerning medical assistance or hospital care in the event of an accident or illness during my absence. I do hereby authorize Players Edge and its assigns to utilize any and all photographs, pictures, video or other likeness of me, my athlete or anyone assigned guardianship to me or my athlete, as they deem appropriate in its promotional materials or team films.
Cancellation Policy: Memberships cancellations are effective on the last day of the month in which cancellation is received. Membership cancellations must be submitted in writing. Scheduled sessions may be canceled without penalty up to 48 hours prior to the scheduled session. Camp Refund Policy:  A Players Edge Credit will be issued for future camps or clinics for any Players Edge Outdoor Summer camp canceled due to inclement weather which can be used any time during the next 12 months. No refunds will be issued for camps missed due to inclement weather. If you wish to cancel a camp, you must notify Players Edge in writing 15 days prior to the first day of that specific camp. A $35 processing fee will be charged on all refunds. Package/Clinic Refund Policy: Packaged sessions and Clinics offer the ability to re-schedule up to 24 hours prior to the scheduled session or clinic, rescheduling is only offered on an as available basis and if re-scheduled prior to the package end date. 

STATUTORY NOTICE AS TO BUYER'S RIGHTS. ANY PERSON THAT AGREES TO THIS ON-LINE CONTRACT IS SUBJECT TO ALL CLAIMS AND DEFENSES THAT THE DEBTOR COULD ASSERT AGAINST THE SELLER OF GOODS OR SERVICES OBTAINED WITH THE PROCEEDS HEREOF, RECOVERY HEREUNDER SHALL NOT EXCEED AMOUNTS PAID BY THE DEBTOR HEREUNDER. 

Three Membership Types to choose from:

Month-To-Month Membership- $99 Initiation Fee, $32 per month (Primary Member), $5 per month (Each Add'l Family Member), payable in advance each month until I choose to cancel.
Six Month Membership- $0 Initiation Fee, $32 per month (Primary Member), $5 per month (Each Add'l Family Member), payable in advance each month, for a minimum of Six Months.  My membership will convert to 'month-to-month' and I can choose to cancel anytime after the sixth month.
College Athlete Weekly Membership- $0 Initiation Fee, $25 per each 7 Days. Weekly registration required.
